What this site is

This site lets New York Medicaid members apply for food support through the Social Care Network and track an application. It is a way to apply; the network decides. Submitting an application is not a promise of food boxes or any other service — eligibility is set by New York State and confirmed by a Social Care Navigator after the state's standard screening. Fine Care Pharmacy is a contracted food-support provider; it does not decide eligibility.

No cost, no strings

Applying and receiving services through the network never costs anything. You do not need to fill prescriptions at Fine Care Pharmacy to apply or to receive food support, and choosing another pharmacy never affects your eligibility. Nothing is sold through this site and there are no refunds because there are no payments.

Your responsibilities

  • Give accurate answers about yourself and your household. Knowingly false statements to obtain a Medicaid-funded benefit are a crime under New York and federal law.
  • Apply only for yourself or for a household member you are entitled to act for.
  • Keep your reference number private; anyone who has it can see your application's status.

To the extent New York law allows, the pharmacy is not liable for losses caused by your use of, or inability to use, this website; nothing here limits rights that cannot be limited by law, and nothing changes your rights under the Medicaid program.
